QLogic offers a comprehensive top-tier medical program pre-bundled with additional benefits (see below). Several options are available. Depending on work location, employees may choose from:
- Preferred Provider Organization (PPO) plan
- Health Maintenance Organization (HMO) plan
- Point of Service (POS) plan
- Traditional Indemnity plan

QLogic offers a comprehensive dental program that covers preventative care, basic and major services and orthodontia.
|
|
|
QLogic provides all regular, full-time employees with company-paid Life and AD&D insurance that is equal to one times the employee’s annual salary.
In addition, employees have the option of purchasing Supplemental Life and AD&D insurance as well as Dependent Life insurance coverage at minimal cost.

Short-Term Disability
For periods of short-term disability (less than 90 days), QLogic employees are assured continued income, provided eligibility requirements have been met.
Long-Term Disability
QLogic provides its employees with long-term disability insurance, which protects against the loss of regular income during periods of extended disability.

Voluntary plan which provides a lump-sum benefit payment if the employee or their covered dependents experience one of the covered conditions.

Health Care and Dependent Care Reimbursement Accounts are available to allow employees to set aside money on a pre-tax basis to cover eligible expenses.

|
The Group Legal Plan is a voluntary plan that gives employees and their dependents access to network attorneys who will provide covered legal services for no additional cost.
The plan also features an out-of-network component where non-plan attorney fees are reimbursed based on a set fee schedule.

|
Employees are eligible to participate in the QLogic Retirement Savings Plan [401(k)].
|
|
|
All active employees, except those on a commission plan, are eligible to participate in the QLogic Incentive Plan. This plan provides cash incentive at the end of each
fiscal year based on company and individual performance.

Employees may participate in the QLogic Employee Stock Purchase Plan which allows the purchase of QLogic stock at a 15-percent discount via payroll deduction. Employees
are eligible at the first enrollment period following their date of hire.

QLogic recognizes the value of time off for vacation, personal business, sick days, religious holidays or emergencies. PTO provides employees with the flexibility to manage
their time off from work based on individual needs. Employees are eligible to take PTO after a minimum accrual. Hours accrue on a pro-rated basis from the day of hire.

QLogic observes 11 company-paid holidays per year, including a shutdown over the December holidays (contingent upon business requirements).

QLogic provides educational assistance for full-time employees for pre-approved job or degree related classes. Reimbursement
includes tuition, registration, books and lab expenses.

QLogic provides the Employee Assistance Program, a 24-hour help line establised to assist employee and their household members with resources such as:

Cash rewards are paid to eligible employees who submit qualified referrals for candidates who are hired by QLogic, and fulfill program requirements.

Employees may purchase computer systems for home use, with up to two years interest free via payroll deductions.
